BO4 had a campaign, but it was going to be some kind of co-op thing. Testers played it and absolutely hated it. It was scrapped and they decided to make a br since it was the height of br's popularity. They made Blackout in like 6-7 months. I think it's pretty incredible that they made such a good br in so little time.
